Complete the waiver form at http://international.utsa.edu/images/uploads/Sp-Sum 2011 HIW.pdf and attach the program summary that you will receive after purchasing one of the appropriate plans below and then submit to Office of International Programs (OIP).

Study Abroad Insurance

Suitable for students enrolled in Study Abroad program outside USA. Purchase online to obtain insurance ID Card & program summary.

$100,000 limit per plan life
Medical Evacuation: $500,000
Repatriation: $50,000

International Student Insurance

Suitable for F1/F2, J1/J2 visa students enrolled at UTSA. Purchase online to obtain insurance ID Card & program summary.

Choose $50,000 or $250,000 per incident.
Medical Evacuation: $50,000
Repatriation: $25,000

Evacuation Repatriation Insurance

Suitable for Research Assistants & Graduate Assistants seeking strictly Evacuation & Repatriation coverage (no medical benefits). Purchase online to obtain insurance ID Card & program summary.

Medical Evacuation: $100,000
Repatriation: $20,000
